Piles During Pregnancy – Causes, Treatment and Prevention

Piles During Pregnancy – Causes, Treatment and Prevention

Piles during pregnancy are extremely common — affecting up to 35% of pregnant women. Understanding why piles occur during pregnancy and how to safely treat them is important for every expecting mother.

Why Do Piles Occur During Pregnancy?

  • Increased blood volume: Pregnancy increases total blood volume, causing veins to enlarge including rectal veins
  • Uterine pressure: The growing uterus puts pressure on the inferior vena cava, reducing blood return from the lower body and causing rectal vein engorgement
  • Hormonal changes: Progesterone relaxes blood vessel walls, making them more prone to swelling
  • Constipation: Very common in pregnancy due to hormonal changes and pressure on the bowel
  • Straining: During labor and delivery

Symptoms of Piles in Pregnancy

  • Rectal bleeding (bright red blood)
  • Itching and discomfort around the anus
  • Pain, especially while sitting
  • Swelling around the anal area

Safe Treatment Options During Pregnancy

Conservative Management (First Line)

  • High-fiber diet to prevent constipation
  • Adequate water intake (8-10 glasses daily)
  • Warm sitz baths for relief
  • Cold compresses to reduce swelling
  • Safe topical creams (consult your doctor first)

Safe Medications in Pregnancy

Only use medications approved by your doctor. Many topical creams are safe in pregnancy. Oral medications should be taken only on medical advice.

When Surgery May Be Needed

Surgery is generally avoided during pregnancy. Most piles improve after delivery. Surgery is only considered in severe cases with thrombosis or heavy bleeding.

Prevention During Pregnancy

  • Start high-fiber diet early in pregnancy
  • Stay hydrated
  • Do gentle exercise like walking or prenatal yoga
  • Use a pregnancy pillow to avoid prolonged sitting on one position
  • Respond promptly to bowel urges

Post-Delivery

Many pregnancy-related piles resolve within a few weeks of delivery as hormone levels normalize and uterine pressure is relieved. Persistent piles after 3 months post-delivery should be evaluated and treated.
