Understanding Anal Fistula

Classification of Anal Fistulas
- Intersphincteric: Passes between the internal and external sphincters (most common, 70%)

Treatment Options for Anal Fistula
1. Fistulotomy (Laying Open)
The fistula tract is surgically opened and left to heal from the inside out. Effective for low, simple fistulas. Risk of minor incontinence with high fistulas.
2. Fistulectomy
The entire fistula tract is excised. Provides tissue for histopathology. Suitable for simple, low fistulas.
3. Seton Technique
A suture (seton) is placed through the fistula tract. Used in stages for complex, high fistulas to gradually cut through the sphincter while allowing healing.
4. LIFT Procedure (Ligation of Intersphincteric Fistula Tract)
The fistula tract is ligated in the intersphincteric space. Sphincter-preserving technique with good success rates for transsphincteric fistulas.
5. Laser Fistula Treatment – FiLaC (Most Advanced)
Fistula-tract Laser Closure (FiLaC) is the most modern treatment. A radial laser fiber is inserted into the fistula tract and laser energy seals the tract from within.
